Healthcare Coverage Form

On November 3, 2017, Massachusetts Governor Charlie Baker signed legislation (H.B. 4008) requiring Massachusetts employers with six or more employees to prepare and file a new health care reporting form created by the Massachusetts Division of Medical Assistance within the Executive Office of Health and Human Services. Once available, employers must annually complete and submit the form under oath and disclose whether they offered to pay or arrange for the purchase of health care insurance. Employers that do pay or arrange for purchase must include information on the form about premium costs, benefits offered, cost sharing details, eligibility criteria, and other information the division deems necessary. Employers that knowingly falsify or fail to file this form are subject to penalties between $1,000 and $5,000 for each violation.

The law is effective February 1, 2018; however, the division has not yet made the form available.
